You can find all the details regarding the accommodation of a person in our home in the attachment.

In order for a person to be accommodated in the Home, the following documents must be submitted, which you can find on our website.

• A certificate from their primary care doctor regarding their health condition, and
• Accommodation request form (a template that can be obtained at the Home or downloaded from the website).

The required documentation can be:

• sent by mail,
• or sent to the following e-mail address: sjurajderbitola@hotmail.com.

We do not sign the accommodation agreement with the person being cared for, but with an individual who, on their behalf, will accept the terms of the agreement and commit to covering the monthly costs, regardless of the type of personal income of the person being accommodated.
